To complete the enrollment process, families will be asked to bring the following COMPLETED documentation to the school:

Enrollment form
Home language survey
Student medical history
Parent information
List of approved individuals for check-out or pick-up
Verification of certified (State) birth certificate
Proof of residency (current gas or electric bill, rental, lease, or purchase agreement, and a valid driver’s license or photo ID
Current immunization record
Withdrawal form from the previous school (if applicable)
Special education records (if applicable)
Free and reduced lunch application
